Bank Transaction Export Summary Report; only works for Australia and New Zealand

The Bank Transaction Export Summary Report only works for Australia and New Zealand. For other countries, e.g. Fiji, Papua New Guinea, the list of columns to include is not built, thereby meaning the report fails.
