Summary Page scripts; add hyperlink to open the Service Log in the History section

A hyperlink to the Service Log has been added to the Summary Page "History" block for Bank Account Enquiries and supported Credit Bureau Enquiries.

The "History" block has a new column named "Service Log" with the Pk of the Service Log, this Pk is also a hyperlink to the Service Log form.

The following built-in Summary Pages have been updated:

"SummaryPage_BankAccountEnquiryReport_BankStatements" has been updated to version 1.02 on 28/05/2015.
"SummaryPage_BankAccountEnquiryReport_CreditSense" has been updated to version 1.07 on 28/05/2015.
"SummaryPage_BankAccountEnquiryReport_DecisionLogic" has been updated to version 1.06 on 28/05/2015.
"SummaryPage2_CreditEnquiryReport_VedaConnectAU" has been updated to version 1.00 on 28/05/2015.
