Workflows; Version 2 Summary Page shows multiple instances of the same Outcome

If a Question or Item Group type Workflow Item defines multiple Outcomes with the same code, these were being displayed on the Version 2 Summary Page.

This was incorrect. The list of outcomes should not repeat outcomes, e.g., if an Outcome of 'A' is defined multiple times (to perform multiple actions), it should only have appeared once.
