Workflow Types form; New 'Summary Script' page added with options for Version 2 Summary Pages

A new 'Summary Script' page has been added to the Workflow Types form.

The 'Summary Page' Script previously defined on the 'Options' page has been moved to this page and a series of options added.

These options are used by the built-in Workflow Summary Page (Version 2).

The Workflow Type details report has been updated.
