Workflows form; Items page; Overlay icon for completed item is inconsistent

The overlay icon for completed Workflow Items was inconsistent with the icon defined against the Item Outcomes, e.g.:

• The "Neutral" icon was never used as an overlay
• The "Failed" icon (the no entry sign) appeared on the Items tree view as the "Invalid" icon (the white cross on a red background)
