Task Manager; Monitored Accounts folder crashes if Monitor Categories enabled in Global Settings

If, when the Task Manager folder is first opened, Global Settings dictate that only Monitor Category A is in use (Accounts, General) then columns in the Monitored Accounts grid will not have been created for Monitor Categories B and C.

If Global Settings is then opened and Monitor Categories B and/ or C enabled then moving to a different Monitored Category folder (or refreshing the existing one) causes the Task Manager to crash with an unhandled exception, e.g.:

An unhandled exception has occurred.
