Account Refinance wizard; fails to run from the Account menu

The Account Refinance wizard fails to run from the Account menu. This causes an error "An unhandled exception has occurred. Object reference not set to an instance of an object.".

Note, you can still Refinance an Account via the Refinance Action on the Account form.
