Ad-Hoc Query Results form; Window caption simplified

The Ad-Hoc Query Results form's caption has been simplified.

Instead of being "Ad-Hoc Query Results" followed by the full Query text it is now simply "Ad-Hoc Query Results 1", "Ad-Hoc Query Results 2" etc.

This means the Window's name under the Window menu is shorter and easier to see. It also means when merging queries the Query Name to select is simpler.

In addition the Merged Query Results form now includes a numeric suffix to help identify the form, especially if this is used again in a further Merged Query.
