Credit Enquiry wizard; No heading showing on first page and Client details not populated when run from Application Shortcut

No heading was appearing on the first page of the Credit Enquiry wizard. Also, the Client details are not populated when moving past the first page if the form is opened from an Application Shortcut such as:

FormShow?form=CreditEnquiry&clientId=C10000

The 'Back' button was also enabled on the first page of the wizard and clicking this would cause finPOWER Connect to crash.
