Credit Enquiry Add-On; Credit Enquiry form, Service page; Service Name now displayed

The Service dropdown list on the Credit Enquiry form lists the Service code but did not display the Service Name.

The Service Name is now displayed in the dropdown list and alongside the Service control that displays the Service code selected.
