Statement Change wizard (and others); tweaks to 'Notes' details

The "Notes" details saved for Statement, Credit Limit and Minimum Payment changes is made up of the Authorised by name and Reason. The text saved has been changed to include " around the "Authorised by" name.

In addition, the Statement Change business layer object, finAccountStatementChange, no longer includes a separate "Notes" property as this was never used.
