Account Monitor Category History form; Now used Monitor Category colours to colour grid rows of show icon

The Account Monitor Category History form now uses the colours defined on the Monitor Category to colour the grid rows or show a coloured icon (based on the User Preferences, User Interface, Colour grid rows setting).

The grid on the Accounts form, Status page, History tab now also has an 'Icon' column and will colour the row as per the Account Monitor Category History form.

NOTE: Both of the above mentioned grids will need to be reset to show the new 'Icon' column (displayed if the 'Colour grid rows' setting is not checked).
