Account form, Payments Summary Page; Minor tweaks made to HTML

The Payments Summary Page on the Account form has had two minor tweaks made to HTML.

1. Current Payment Arrangement "Reason" now support multi-line text.
2. Bank Matches now fixes the left icon column to 20 pixels.

In addition the Payment Arrangement Summary page now includes the "First Payment" date.

The following built-in summary page scripts have been updated:

"SummaryPage_Account_Payments" has been updated to version 1.08 (27/08/2012).
"SummaryPage_AccountPaymentArrangements_PaymentArrangementSummary" has been updated to version 1.04 (29/08/2012).
