Reports; Queries adding Monitor Category columns when not in use

Various queries were adding the three Monitor Category columns without checking if they were defined as 'In Use'.

The settings to define whether a specific Monitor Category is to be used can be found on Global Settings, Accounts, General page.

The following queries have been updated:

• Account List
• Transaction List
• Accounting Ledger List (Accounting Ledgers Add-On)
