Publishing Account documents; Fee charged twice if document VBA sets Fee and document is unpublished and published again

When publishing Account documents a fee will be recharged, ie double charged, under the following circumstances:

1. Fee is set in the document VBA using LogPublishItem.Fee property.
2. Document is published, then unpublished (gives warning to review fee charged) then published again.

The second publish will recharge the fee if the Fee is set in the document VBA.

The LogPublishItem has been changed as follows:

1. The property Fee will always be set to the Fee either to charge, or if already charged, the Fee that was charged.
2. A new property FeeAccountTransactionPk has been added. This is the AccountTransaction Pk that the Fee has been charged under (if already charged), or 0 if not charged.
