Security Statement Item Logs; inconsistent numbering

The numbering in the heading of a Security Statement Item Log was inconsistent with what displayed on the Log Details report.

The heading on the Item Log form displays the Id (primary key) of the Log record.

The heading on the Log Details report now displays both the index of the Security Statement Item (as per the grid on the Security Statements form, i.e., starting from 1) as well as the Log Id.
