Account Transfer wizard, Transfer Details page; Reference and Notes added

The transfer wizard now allows the entry of separate Transaction Reference and Transaction Notes for each of the 'From' and the 'To' Account transactions.

The Reference and Note defaults are created by:

1. Using the Transaction Type defaults, then the Element defaults
E.g.; No transaction default then will look at the Element default.

2. A Transfer message is added to the default notes.
E.g.; Transferred to [AccountIdTo]
